Sucess Stories - JAke
Rebuilding Confidence and Returning Home
When Jake, name changed for confidentiality, joined our home, he was subject to a DoLs order and required 2:1 staffing in a solo placement due to the complexity of his behaviours and risks. Before this, he had been living with his parents, who cared deeply for him but had struggled to manage his increasingly complex needs without specialist support. On admission, Jake had limited engagement with education, irregular routines and difficulties with emotional regulation, causing significant instability in daily life. Our team recognised he needed consistency, structure and positive role models to build confidence and develop greater independence.
Establishing Stability and Routine
In the early stages of Jake’s placement, the focus was on building trust and introducing predictable daily routines. Staff worked closely with him to create structure in his day, supporting him to develop healthy habits and activities that gave him a sense of purpose.
One of the first successes was helping Jake establish regular routines, including attending the gym and participating in physical activities, which quickly became a positive outlet for him and supported his overall wellbeing.
Re-engaging with Education
Education was another key priority. With patient encouragement and tailored support from the staff team, Jake gradually began to re-engage with learning. What initially started as small steps developed into consistent participation in education, helping him build confidence and a renewed sense of achievement.
Reducing Restrictions
As Jake’s stability improved, his behaviours became more manageable and he demonstrated increased ability to regulate his emotions. The team worked closely with his social worker, the Local Authority, and the courts to review the restrictions that had originally been put in place.
Through clear evidence of progress and careful planning, the DoLs order was successfully removed, marking a significant milestone in Jake’s journey.
Rebuilding Family Relationships
Alongside these improvements, an important part of the plan was to rebuild and strengthen Jake’s relationship with his family. The staff team supported structured and positive family time, ensuring visits were planned and supported in a way that felt safe and meaningful for everyone involved.
Over time, these family relationships grew stronger and more stable.
A Successful Return Home
After just over a year in placement, Jake had made remarkable progress. With the right support networks in place and improved stability in his life, the decision was made for Jake to transition back home to live with his parents.
The move was carefully planned and supported by the staff team to ensure the transition was smooth and sustainable.
The Outcome

Jake’s journey highlights the impact of consistent care, positive routines, and strong partnership working with families and professionals.

What began as a placement requiring significant restrictions and intensive staffing ultimately became a story of growth, stability, and reunification with family.

Today, Jake is back living with his parents, engaged in positive routines, and continuing to build on the progress he made during his time with us.

A powerful reminder that, with the right support, young people can successfully return to the people who matter most to them.
